Rapid Rehousing Program - Transitional Housing
The SWAN Rapid Rehousing Program is a transitional housing program to help victims of domestic violence find permanent housing through short- or medium-term rental assistance. A Housing Care Manager is available to provide supportive services and case management to help victims complete assistance applications for Medicare, Medicaid, SSI, SNAP, TANF, employment resources, educational resources, transportation, and safety planning.
